Add missing fields to NameId #17

I needed at least the Format attribute for the NameID, so I've added the other attributes according to saml-core-2.0-os page 13 as well.

Ah, sorry, thanks for noticing!

I have updated the PR to include documentation for the new fields and moved the existing documentation to the right field.
