recurring, intermittent troubles w/ shimming, happened again on 9/19/2016 w/ EPSI sequence, global shim not working? 3D shim not working? manual shim starts with very poor fwhm values, takes a long time to adjust back to reasonable values

motion causing poor slice placement?
