Introduce lines.txt, changes to routes.txt #4

These changes will appear in the MBTA's GTFS on Thursday, December 20, 2018.

lines.txt (new file)
route_desc in routes.txt (new convention, changed values)
route_fare_class in routes.txt (new field)
line_id in routes.txt (new field)
listed_route in routes.txt (new field)
