Custom condition effect icons do not appear on tokens.

Describe the bug After updating to v1.4.5 from v1.4.1, the custom conditions that are added into Condition Lab do not appear on the token when activated. Only the default system conditions effect icons appear on the token.

To Reproduce Steps to reproduce the behavior:

1. Go to Condition Lab
2. Create new condition
3. Assign status effect via the token HUD
4. Icon does not appear on the token

Expected behavior Icons should appear on the token just like the default conditions do.
