Speed up things #6

  1. Efficient implementation of parsing phenotype matrix. e.g. reduce a 100x173 matrix (100 patient, 173 disease) to size of 6x2.

  2. Added Joe to the contribution list in the DESCRIPTION

  3. Updated the citation information

  4. Included unittesting examples
