Add a FAQ #48

I've seen these two questions come up a whole lot on Slack. This could save some time, and is checked into source control so it can change as the library does.

Not sure of the right balance between discoverability and meta-help here, so I made a separate file. It may belong in the README instead.

Feel free to edit this however you like. (in particular I'm using the royal "we" here while recognizing that it's mostly you doing the work, @mdgriffith.) Or just reject it entirely, whatever. I won't be offended. :)
