Documentation update #30

We need to update the README to prepare for the public testnet.

cli documentation:

[x] configuration details (how to provide addresses for rpc cluster, datastore, etc)
[x] translator / ingestion invocation (needs to be updated to include translator id after #17 is merged)
[x] document get command
[x] add docs for --use_ipfs flag

"internals" documentation:

[ ] outline relationship with indexer project
[ ] document translator output format (in-progress: #43)
