get duplicate canonical ref from grpc metadata #86

pulls the reference key from the gprc error's initial_metadata instead of parsing the error string. Works with transactor master (deployed on testnet).

works as advertised on the devnet.

I'll merge this for now, but let's try to keep stuff like this in release branches from now on.
