Make MKA & Presentation Chapters for transcription discs #310

  1. created 'make_make.sh' to create MKA files from WAV files
  2. updated 'mmfunctions' to accept MKA files and to loop through INPUTFILE to identify Presentation Chapter Edition Entries
  3. Minor update to documentation in 'makemkvchapters'
