Update CHT reference documentation on partner pages. #1417

There is some CHT reference documentation on interoperability that is outdated:

  1. OpenHIE partner page
  2. The OpenMRS Integration Catalog

For the OpenHIE page we can include the 130k users milestone and the implementations we have in production. cc @jeffmedic @boussokane
