Reference app URL incorrect after latest update

We have updated the reference application documentation from ANC to Maternal and Newborn Health. The URL file path still shows "apps/examples/anc/". Update the anc.md file and the anc folder to match the revised documentation. Proposed file path "maternal-newborn". Best way to do this may be through a "gitmove" to maintain history.

@abbyad I tried to rename the .md file and folder, but it shows up as a complete re-write. So, it looks like we need to use a gitmove if we want to maintain history. I unfortunately, don't know how to do this. Do you want to take this on, or even better, give me a tutorial?

For short MNCH is commonly used, and we can add the "C" part later.

As for moving the files, we can walk through using git mv.

In the meantime, you can also try running git add -A . [1] in your directory, and then seeing if it recognizes the changes as a move instead of deletions and new files.
