feat(#114): Create OpenMRS Mediator #115

Change mediator to accept unformatted documents from CHT and map them to FHIR resources that will be accepted by OpenMRS in the mediator.

@witash, is it expected that in open him transactions, I'm seeing a lot of calls to GET /openers/initialsetup?
