add endpoint for creating patient #9

This creates an endpoint in the mediator that can be called by the CHT to create a new patient. This addresses https://github.com/medic/interoperability/issues/5

I just discovered this can be done with outbound push with mapping so there might be no need for this to be in the mediator.
