84 sql exporter and ingress

• adds optional sql exporter for prometheus to the helm chart
• adds optional ingress configuration for medic infrastructure
• adds 'pending' and 'updated_at' to couch2pg progress table
• adds optional sql exporter config with suggested metrics

some things to note

• sql exporter is for k8s deployments only currently because using helm template to populate db name and schema in collector config. can work for docker compose as well, but would need to move collector config somewhere else and set env vars
• sql exporter can also be deployed as part of cht-watchdog if its allowed to access the db, in which case the collector config would need to be copied/pasted. Not sure how to get around this.
• the Pouchdb changes api doesn't return 'pending' directly, so need an additional API call for it, to get update_seq and subtract current sequence. adds to latency, I don't think enough to be significant
• using update_seq - current_seq is not accurate, everything I read about sequence numbers in couchdb says not to do this. But, this since is only an estimate for monitoring, it may still be useful.
• dbt metrics depend on this branch to get results in the db, but even if tables are not there its ok will just report scrape errors and the couch2pg metrics will still work

@njuguna-n Im going to merge this; it has some changes to couchdb_progress which will break couch2pg if using an existing db. if so, run ./deploy.scripts/upgrade/upgrade-84-monitoring.sh <kubectl|docker> <username> <dbname> [namespace] which will automatically run the ALTER TABLE to add them
