Add configuration which disallows login as admin #101

Once we create the user accounts for KE User Managers via https://github.com/medic/cht-user-management/issues/79, we don't want anybody to use the medic super user account anymore.

On Feb 21, we announced:

Everyone's usernames will be present in the data they create. This is an important step in our road toward an auditable user creation process with explicit recorded approval. I propose that on Apr 1, users will no longer be able to login to the tool as admin (username medic).

This tracks disallowing login via medic once accounts are created and distributed
