Cannot login as admin to select instances #102

Triggering: User 'medic' does not have a facility_id connected to their user doc

After #100, this is resolved for Migori but still happening for isiolo, siaya, and chis-training.health.go.ke.

isiolo had a conflict on the org.couchdb.user:medic doc within the medic database. In the latest rev, the admin role was removed, but it was present in previous revs. I corrected the issue by resolving conflicts and updating the role to admin.

No-repro for siaya and chis-training
