Devolved user management strategy for eCHIS Kenya instances with multiple counties #106

Today, a user_manager account needs to be connected to a single place.
eCHIS instances with multiple counties need to have multiple user_manager accounts (one per county), or be managed by admin
Training instances like chis-training-2.health.go.ke have ~17 counties

What should the use rmanagement account strategy be for these training instances? We likely need a feature in cht-user-management to support an admin, or an update to the scripts in #79 to support this.

Siaya and Migori are also on a single instance. It'd be nice to have one user account instead of forcing a secondary.
