This PR:

Removes all old files relative to postgres exporter now that it's deprecated
adds compose and config files for burningalchemist's sql_exporter
updates fake-cht dev instance and CI tests to use the new burningalchemist image
updates dashboard JSON to use new field names to calculate couch2pg backlog

per #81
