Update csv to docs #159

Moved documentation from the medic-conf readme that was useful but too detailed for the readme.

I did a pass to clean up content, which should bring it into a better place for what we need now. @SCdF feel free to edit further, or revisit this again when we create tutorials and include this in the new documentation site.
