Update documentation on linking child documents #208

The current approach illustrated in https://github.com/medic/medic-docs/blob/master/configuration/forms.md is causing an overallocation of memory (and crashing on low RAM phone).

Use the identified workaround in https://github.com/medic/cht-core/issues/6136 and update the example(s) in https://github.com/medic/medic-docs/blob/master/configuration/forms.md
