updated based on user feedback:

"Continuing with forms, dealing with images in the CHT took me much longer than expected. I spent a couple hours dealing with pictures not showing up and only having a spinner until I was pointed to this issue (https://github.com/medic/cht-core/issues/6321). At that point, I was able to upgrade my instance to 3.8.1, and my problem was fixed. Stating this in the forms documentation probably would have saved some time."

Closing as this was a bug that has been fixed - all versions in which this did not work are already beyond their end-of-life date for support.
