Medical ID head up displayed upon incoming call #91

It seems related to the way the lock screen notification service is started.

Commit https://github.com/lpellegr/medicalid/commit/863e9a6e3efbbd39d6fa3f3a8f515b95dc46490d reduces the scope of this issue. However, it seems not yet fully fixed.

I think the scenario where the issue happens is identified:

  1. Install the app
  2. Turn the screen off
  3. Turn the screen on
  4. Unlock the lock screen
  5. Turn the screen off
  6. Receive an incoming call

At this stage, a notification head up appears for Medical ID.
