CodeSystem ATC-WHO is missing #61

The CodeSystem "ATC-WHO" (canonical http://www.whocc.no/atc) is not represented in the package and not available in FHIR format on the Internet.

A strategy for converting the CodeSystem from the native format, or a placeholder CodeSystem should be created should converting the CodeSystem not be possible due to technical or license reasons (in this case, the CodeSystem and the ImplementationGuide should describe how to acquire the master file to implementers).

The ATC Index for WHO is purchasable from https://www.whocc.no/atc_ddd_index_and_guidelines/order/, but the licensing terms are unclear if representing the resource as FHIR is permitted:

WHO Collaborating Centre for Drug Statistics Methodology, Oslo, Norway. Use of all or parts of the material requires reference to the WHO Collaborating Centre for Drug Statistics Methodology. Copying and distribution for commercial purposes is not allowed. Changing or manipulating the material is not allowed.
