Make the upload file button on document and roster forms the same #251

Right now, the button to select a file on documents/add and rosters/add is different. We need to make the button the same on both forms, so both buttons look like the button on on rosters/add.

As discussed with @mitchellias, I will style the form entirely in the same style as /rosters/add
