Extend estimates for standing dead trees that fall over #15

Trees marked STANDING DEAD are surveyed until they fall over, at which point they are supposed to get one more survey record with STANDING_DEAD_CD == 0.

When this happens, we want to extend the interpolated values for HT/DIA to the midpoint of the resurvey period in which the tree fell over. (Currently the values stop in the last survey year in which it is standing dead).
