Add "required" label for all required fields in forms

A red "required" text would be good to have (probably right below the help 
text).  This way, users know what is needed before they encounter the error.

Added "required" and "(optional)" to all the fields on all the forms that I 
could find (and verified that the validations were there).  Tell me if I missed 
anything
