Submitting custome ItemTypes (and ItemAttributes) in new Item form

We need to allow adding an Item with a custom ItemType (DVD, etc.) and 
ItemAttribute (Smelly, etc).

These types and attributes should be created with their hidden field selected 
and it should be possible to add multiple custom ItemAttributes (which can 
probably be done after I fix the way we show ItemAttributes to support 
maintaining the form data after a failed submit -- Issue 105).

You can assign this issue appropriately
