Copyright on all notebooks #73

Per our proposal, we need to release as Apache 2 for code. The best license for text materials seems to be CC BY-SA because various images and apparently some text has been adapted from Wikipedia.

Here is a template for our copyright statement, with credit for each author:

Copyright 2020 Andrew M. Olney and made available under [CC BY-SA](https://creativecommons.org/licenses/by-sa/4.0) for text and [Apache-2.0](http://www.apache.org/licenses/LICENSE-2.0) for code.

We can "lock" this top cell in the notebook using the following cell metadata:
