Resolve issues around 'db in dirty state' #3

Running more than one migration with mattes/migrate is currently returning this error, 'Dirty database version N. Fix and force version.`

I had to manually clear the data and re-run the migrations (eg. up 2)... not exactly workable in production. We might want to move away from this library, or diagnose and document the error and workaround.
