My LG OLED running WebOS 3.0 backlight control does not work. It will actually pause or play the content on my Apple TV instead. My other LG OLED running WebOS 4.0 works flawlessly with the same setup.

I would guess that WebOS 3.0 does not support that feature. You could also enable the debug log https://github.com/merdok/homebridge-webos-tv?tab=readme-ov-file#troubleshooting and post the debug log from when you try to use that switch so we can see what is actually happening.
