Final proof read #567

A bunch of little minor changes from a final cover-to-cover proof read. (I'll be careful to be aware of and not create conflicts with #560).

I haven't finished reviewing the appendices (I've only done the solutions at this stage), but I think it would be good to review and merge this PR now before tackling #568.
