Add outline of action steps for chapter 12 #68

This is not looking like a very big chapter, but I think that's ok. It will be tempting (for me) to get wordy on this topic, and I tried not to go overboard in the description at this early stage.

I am unsure about the order and flow of the material. I'm leaning towards starting with GitHub Issues and moving on to discussing how they come together for project management with Milestones / GitHub Projects, because it feels like the latter would make more sense if the purpose and scope of Issues are explained first.

I think this does an awesome job highlighting important points in project management and associated tools that make this possible in a more formal sense. My proposal in #100 helps sidestep the issue of how to structure this content by removing the chapter and adding its material to 10-community-involvement and 13/14-team-based-development. This means issues are generally discussed all together in chapter 10, but using issues for planning/milestone/etc is in the last chapter.
