I'm using VideoIO11 ver 3.3

4) I am not sure if this is a VideoIO issue or flash issue, there are some odd,
rare situations when the option to allow access to webcam and mic is not
clickable - it shows up and you have to click it ten time to select an option.

Found some relevant posts regarding the bug in Flash Player.

Feel free to post any potential solution. My gut feeling is that the Flash
Player is trying to write to some location in the file system (or registry),
fails due to permission, and does not handle any clicks due to that. But this
is just a guess!
