Flatten section hierarchy #6

Promoted some subsections to top-level sections. New TOC looks like this:

  1. Notational Conventions
  2. Terminology
  3. Introduction 3.1. Issuance & Presentation
  4. Creating an SD-CWT
  5. Creating a Key Binding Token
  6. Validating an SD-CWT
  7. Examples 7.1. Minimal spanning example
  8. Security Considerations 8.1. Random Numbers
  9. IANA Considerations etc.

Also added one missing closing triple tick
