Document 02v4: Add underline where needed #127

Attending to this query:

Type 1: really textual content missing underlines (random underlines in text). => please fix in markup.

Fixes https://github.com/metanorma/mn-samples-plateau/issues/72

I checked both documents: 01 and 02. Only Document 02 needed underline markup.

These work because they end at a line break:

These don't work because they are bound within the sentence, they need double hashes to work.
