Adding git pt-open which opens the PivotalTracker story page for the current branch.

Khiet thanks for this, few notes:

Please rename this command with git pivotal-open. We would like to not have abbreviated names and commands can always be aliased per user preferences
Please provide an option parser https://github.com/mezis/git-whistles/blob/master/bin/git-select#L89 method that outputs the use case of this command. Also it should output the help message when doing git pivotal-open -h

I think we are nearly there.

I would suggest changing the readme and the output of the command to something like:

Open pivotal tracker for the current branch story id assuming branch follows the format xxxx-id or for the given story id received as option.

What do you think ?

I just updated the README and the comment in the command. I'm not in favour of adding the description in the -h output when the other commands or gems I can find don't.
