Felipe and I were reviewing the interface and we inserted an accession, playing a lot with values in the different tabs. when we had removed all that was unintended, we could click on OK, the interface complained about an assertion failing, relative to date of note.
but the accession had been inserted, this was totally not made clear to the user.
