MySQL support for joined updates
CTEs correctly populate scopes for .update() (fixes #36)
Cte.as_ no longer an extension - fixes lack of import prompt for as_ in .with() (closes #35)
coalesce now supports literals directly e.g. coalesce(x, value(0)) can be coalesce(x, 0).
