Create a DocBook set of templates #16

A !DocBook set of templates would be useful for including in books. Use xinclude to create one large document from all the individual documents.

There is a bug with the files in the top-level directory. There is also no DLM template yet. I think that a organization similar to the LaTeX style might be useful, where there is just one top-level file and one file included for each source file.
