improves: EP 41's dictation, speaker identification and formatting #114

Closes #109

So I ran this through a parser after completing the following (parts were based on listening, others reading to correct broken sentences / thoughts) which formatted it to leverage a max of 80 columns, hence why every line is claimed to be changed.

Corrections:

Capitalized, and corrected the identification of speakers
Corrected broken sentences when a speaker took a moments pause which resulted in transcript believing that was the end of their statement
