Case insensitive search, vol 2 #18

Add case-sensitive, ignore-case and smart-case options, and change matching behavior based on that.

Todo:

[x] cycle through modes with a keyboard shortcut (which shortcut? also: update search matches when doing so)
[x] update cli arg descriptions
[x] ~~update cli arg ordering~~ - done later, overhaul ordering of all args
[x] update README

Rebased on to new master, supersedes #6.
