Fix deprecations #213

Supersedes #192, #210, and #211 by applying all three, as well as:

Seems running the docs build in https://github.com/mharris717/ember-cli-pagination/pull/213/commits/f5f054d3bd34d43ddbb4656b2e5c076eaeb216aa has caused several README-only changes to be lost as those changes were not in the ./docs source files

If the reviewer can let me know which changes should be added back to the source files, I can do so.

Should probably be a HTML comment at the top of the README like <!-- THIS FILE IS AUTOGENERATED FROM THE ./docs FILES, MODIFY THEM INSTEAD -->
