Definitions from some packages listed here: http://tex.stackexchange.com/questions/1863/which-packages-should-be-loaded-after-hyperref-instead-of-before

I only included those that explicitly mentioned a loading order.

There are some more mentioned in ftp://ftp.tex.ac.uk/tex-archive/macros/latex/contrib/hyperref/README.pdf (see titleref, varioref, arydshln and ltabptch) but I couldn't figure out how to best express the rules for them.

Thanks for the contribution! I'm pretty busy, but I'll try to have a closer look at your contribution this week.

In the meantime, apart from the comment above, I note that you're having hyperref load before a number of other packages. I was under the impression that hyperref generally has to be loaded late.

Also ---and I know I haven't followed this rule myself so far; I will--- if a rule comes from a source other than the official documentation of a package (for example, a TeX.se post), I think we should explicitly justify the rule in the comments, i.e., a short summary of the TeX.se post.
