Patch to better handle merging of contacts on first run #6

As you note in the README file on the first run with a new set of accounts to 
sync, eg. on your very first run, or after you have been running it a while and 
add another account into the mix, you will get duplicated contacts, even if the 
names match.  This patch avoids this, by checking for duplicate names and using 
the UID from a pre-existing contact if possible, please merge it into your code 
if you think it is OK.  Thanks.
