Improved template for new issues #151

To the template for new issues, we might add: 

What type of connection to the Internet did you have at the time of the issue? 

What upload and download speeds do you normally associate with that connection? 

Does the remote server to which your issue relates normally provide steady 
upload and download 
speeds? Or are peaks, troughs and pauses noticeable if you use more traditional 
FTP/SFTP clients?

And so on.
