People got new radios around that time. It's most likely the radio code bug that's currently hovering around
We had the same issue in a large scale TvT. Radio working very erratically. Some of us had the TeamSpeak error reported above. Task force version was the last Beta 1-1.0.298. Of course on dedicated server.
errorLevel out of bounds is not an error. It's just a warning for me.

Are there like. Several minutes where it doesn't work?
In that case run a /tfar full
after having tried a couple times.
Also note down who's radio you could not hear. And send me that.
Erratically meaning at random yes. Myself had radio working for a third of the mission, then not working for another third, then working again. Someone had just the short range radio working. Many others like me only voice when the radios were not working. Radios were anprc-152, fadak, rt1523g_rhs, mr3000_rhs. There was not a specific radio not working, all of them didn't work. And the effect was not for minutes but for a long time. For me the radios worked again at respawn, hard to tell about others because it was a large scale event. Not sure if and how I can reproduce to give you the required infos.
This is so old. Might have been fixed?
@SekiAlpha nope. Still happens, but very rarely and not reproducible.
When this does happen what information do you need and does it affect a lot of people at once?
I need output of /tfar full
in TS chat, from the guy who cannot receive the guy sending. Or from both people maybe.
Can put it into a zip archive and just drop in here.
Not really sure if it really helps though. Might only show sympthoms, not causes.. Actually.. Probably doesn't help me at all, will just tell me empty encryption code and that's it.
Well I don't know if that helps, but TS without any mod, some times bugs like that ! Same symptoms! It happened to me once, and I had to dig until find out that had to change some config on my windows 10 to "decrease" this issue.... Well btw, just got Arma 3, and I'm going to test this bad boy, and may can contribute to find out some solution for this even if it's a TS issue!
@saormart what issue are you talking about? The only issue mentioned here is not being able to hear people over the radio and the debug "error" which is not an error it's just a debug message.
I know this issues is quit old, but we had the same problem in our group and maybe found some inside to the problem. For use the radio stopped working if someone died and re-spawned and got a new radio. In most cases the newly re-spawned players could hear each other over short range, while all still alive player couldn't here them, in most cases long range radio still worked. We also found out that if you picked up your old radio from your old body the radio starts working again or a reload of the mission for that client also fixed everything. I hope this can help you find the cause for this annoying bug.
Ola peeps, this is what we are getting from using the /tfar full command nothing from restarting the game to reinstalling TS and TFAR seems to work.
Validating game files does work!
We took this out of the game after trying to test. I hope this helps.
