Does it happen also if you disable dash-to-dock/ubuntu-dock?
I disabled ubuntu-dock and restarted twice. Both times loaded correctly. I think the problem should be ubuntu-dock.
Is it possible to force an icon theme via Settings.ui or appIcons.js?
I didn't understand that the problem was the icon theme. I don't see how the extension would interfere with the icon theme. We do not deal with it at all, so there's no settings or code to change that.
Now the error occurred too in dash to dock. :(
The error occurs only in dash-to-dock/ubuntu-docky.
In the rest of the system the icons are displayed correctly.
Is it possible to include the chosen icon pack option?
I'm not going to let the user customize the icons of the dock: this is the icon-theme's job and it can already be set in gnome-tweaks for instace. The fact that the dock seems not to follow it is a bug, albeit of unknown origin.
If you restart reload the extension (enable/disable it) when the problem occurs, are the icons set to the right them?
Could you check for errors in the log woth this command journalctl -f /usr/bin/gnome-shell
?
Currently having the same issue on Debian "Testing". After saving my nvidia config to /etc/X11/xorg.conf.d/20-nvidia.conf my icon theme began to reset after each reboot.
After reading about the issue here I tried rebooting with the dash to dock extension disabled and my icons didn't reset. Could it be possible that the problem is related to the nvidia config file?

Same issue here on Manjaro with gnome 3.30.
I use the La Capitaine icon pack and it looks like this.
But every time the computer restarts the icons are loaded incorrectly (randomly).
It need to reapply the theme to fix the icons.
In use: Arch Linux Gnome 3.24 Ubuntu Dock 0.7 Dash to Dock (To reduce the size of icons in the Ubuntu Dock)
