this is still mostly a template transclusion that needs editing to be ADR-specific. I will try to get this to a reviewable state ASAP, to reflect decisions made for the already-deployed minter service.

@shreddd @turbomam this is ready for review now. not as thorough as I would like, but "shippable" to me, esp. with the "More Information" links.
