Update docs to not imply the MkDocs dev-server will start automatically #1854

I added an "(if running)" note to the MkDocs dev-server node in the development environment diagram; and added instructions for starting the MkDocs dev-server manually.

I also deleted some obsolete comments from the Dockerfile.
