Lakshmi - Material Requests page #11

  1. Create pages for Material Request (list/create/edit/delete)
  2. Items table will contain the following columns
    • Item (lookup field)
    • Description (auto populated)
    • Quantity
  3. Top section contains the following fields:
    • Created By (readonly)
    • Created Date (current date, readonly)
    • Project (select2 lookup)
    • Requisition Number
